List of templates used for appraisees' reflections on the Core Elements
The list of reflective templates below is designed to aid the appraisee in their reflections on supporting evidence they wish to submit for their appraisal. For further guidance, please consult your appraiser.

Whilst you are asked to reflect on your supporting information, you are NOT expected to use every template offered below! Only those you wish to use and applicable to your needs (you may have documented your reflections elsewhere in the appraisal forms). Remember, the templates are there to aid you as additional supporting information, they are NOT mandatory.
To use the templates, click on the appropriate link. You will be prompted either to open or save the file (.rtf). We recommend you save the file to your computer first, otherwise it may get lost in the temporay folders. Once the template has been saved you should be able to navigate to the folder you saved it to, and open it.
The templates are in .rtf format, so will work with both PC and Mac users. Only thing to ensure is when you have finished filling in the reflections, make sure you save it in the .rtf format instead of other platform specific formats (e.g. .pages, .docx etc).
Once you have completed your reflections, save the file and upload it as part of your supporting information to Form 3.
An online CPD log has been added to SOAR so you can enter your CPD entries directly, rather than using this spreadsheet. However, if you wish to use the spreadsheet instead, please note the following instructions: This Excel spreadsheet is designed to help the user record their Learning Activities throughout the year. Save this spreadsheet to your computer (right-click > Save target as) and work on this as you go throughout the year. Upload the final version to SOAR when the time comes for your appraisal. Make sure you check the worksheet "Examples and Tips" for guidance. Although originally designed specifically for GPs, it can also be used by consultants (just ignore the impact scoring).
