At the end of each appraisal, the appraisal forms are wiped and reset to let you start preparing for your next appraisal - apart from these pages.
The background information is unlikely to change year-on-year so the details provided will remain and you simply need to update as needed.
And unless you change specialty, it is also unlikely your "current activities" will drastically change, so information in this section will also remain. Again, you just simply need to edit as appropriate rather than retyping the same information every year.
