The final section to look at is the self declarations statements on Health, Probity, and Complaints and Compliments.
The format is essentially a series of checkbox confirmations and Yes/No questions, and where prompted you will be asked to provide more information.
For example under the Health statement, if you declared that you were not registered with a GP, you will be asked for more information. Or if you had any illness or conditions which had resulted in the restricting or changing of your professional activities, again you will be asked for more details.
Where applicable, you can use the "Supporting documents" tab to upload more information, for example, occupational health letters or "fitness to work" notes.
Navigate back to "Current appraisal" to go to the next statement, or you can use the back button in your web browser.
Under Probity, you are asked to read and declare any issues and if appropriate, upload any supporting documents. Again you will be prompted for additional information should anything be declared.
Remember to check all the boxes at the bottom of the page.
Under Complaints and Compliments, you are asked to declare firstly if you have been involved with any complaints since your last appraisal meeting. If so, provide the necessary details as requested.
Don’t forget to check the box at the bottom of the page to confirm that all information provided is true to the best of your knowledge.
Additional supporting documents can be added, along with any documented compliments that you may wish to share with your appraiser. For example, patient thank you cards, or kind message of support from a colleague, etc.
