Sometimes, an appraisee might be exempt from appraisal in a given appraisal year. Reasons could range from maternity leave, to extended sick leave, to sabbatical etc. In which case, a blank appraisal is created and a Form 5A is issued in lieu of a Form 4.
Form 5B is used to record non-engagement. This would be despite various and numerous attempts to contact the appraisee and there's been no response.
When a Form 5A or 5B is created, the first thing you need to do is select the correct Appraisal Lead from the dropdown menu at the top - that person is then responsible for approving your Form 5.
Regardless of either Form 5, we advise appraisers to liaise with the Appraisal Lead before initiating these forms (especially 5B).
Do NOT click on Form 5C, this form has been disused and will be removed from SOAR in due course.
