When the appraisal Form 4 is signed, off, your appraisal forms should be reset and wiped to allow you to start preparing for the next appraisal - apart from information under "My practice" and the "PDP Review" section.
Unless you change specialty or health board, information provided in "Background information" and "Current activities" will not differ much year-on-year - so you just need to edit and update the information as you go.
Any incomplete PDP items from the signed off Form 4 will form your "PDP Review" items for the year ahead, you just need to update its status and comment as you go.
Apart from that, all other sections in "Current appraisal" as per above screenshot should be wiped clean for you to start afresh and prepare for the next appraisal.
