So here’s a quick recap of the key points to be aware of:
- Normally it's the appraisee and appraiser who organises the appraisal meeting themselves.
- Once agreed, it is the appraiser who creates the appraisal on SOAR.
- Remember to check for old incomplete appraisals in the appraisee’s history, if there is, liaise with local teams to get it signed off as soon as possible, before adding a new meeting.
- Appraisee completes their appraisal forms, including the uploading of relevant supporting information.
- If there are any incorrect information, such as Recognition of Trainer or Academic Medicine forms, get it resolved BEFORE you submit them to the appraisal.
- When everything is finalised, the appraisee submits the forms to the appraisal for sharing.
- After the appraisal meeting, the appraiser is responsible for drafting the Form 4 summary.
- The appraisee reviews and approves the Form 4.
And finally, the appraiser and appraisee are then asked to complete the Feedback Form 6.
