Self-declarations

At each annual appraisal, doctors are asked to complete the below self-declaration statements on SOAR:

Health
Probity
Compliments and Complaints

These self-declarations are simple straightforward forms with a series of yes/no questions, and where appropriate, you will be asked to provide additional commentary.

Health statement

You are expected to complete a declaration of your health at every appraisal.

Probity Statement

Required at every appraisal.

Compliments and Complaints

A way to identify, confirm and build on areas of good practice, strengths and what you do well.
