Health statement


For the health statement, you will be asked the follow:

  • Are you registered with a general practitioner?
  • Since your last appraisal, have you had any illness or physical condition which results, or has resulted, in you restricting or changing your professional activities?
  • Please confirm that you accept all the professional obligations placed on you in the guidance Good Medical Practice and Serious Communicable Diseases.
  • Since your last appraisal, are you or have you been the subject of proceedings under the GMC’s Health Procedures or similar proceedings of any other professional regulatory body or licensing body within the UK or abroad?
  • Since your last appraisal, are you or have you been subject to medical supervision voluntary or otherwise and/or any restrictions voluntary or otherwise imposed by your employer or contractor resulting from illness or a physical condition within the UK or abroad?
  • Can you confirm that you have considered the GMC advice on immunisation?
  • Confirm that "All the information in this declaration is true to the best of my knowledge".

If you have had a illness which has impacted on your work, a template has been provided before to aid your reflection.

Supporting information

If applicable, supporting information here could include a letter signed by an occupational health physician confirming your fitness to practice if you had been off sick for a period of time.

DVLA guidance on fitness to drive

Road traffic accidents often involve drivers whose fitness to drive is impaired by a medical problem. These have involved impairment due to existing medical conditions, age, driving whilst taking prescription drugs, and drivers who have a known history of drug dependence.

It has been suggested that in these cases there appear to have been missed opportunities for the patient's doctor to discuss their driving status with them and/or report concerns to the DVLA. It has also been emphasised that medical professionals need to be aware of their duty to protect the public and of occasions where the public duty justifies the breach of professional confidentiality and may be particularly pertinent to HGV and PSV drivers.

This is an issue which all doctors are likely to encounter.
