What goes where?


Providing appraisal supporting information is intended to be an educational and developmental process for the doctor/appraisee, allowing you to reflect in depth on an aspect of personal practice each year.

Your supporting information must demonstrate that you are continuing to meet the principles and values set out in GMC Good Medical Practice (last updated in January 2024).

The discussion and summary will be focused on the below four domains:

  1. Knowledge, skill and development
  2. Patients, partnership and communication
  3. Colleagues, culture and safety
  4. Trust and professionalism

The way SOAR is structured, it focuses on the supporting information requirements so you don't need to worry about what documents goes in which domain.  The supporting information requirements are as below:

  1. Continuing professional development
  2. Quality improvement activity
  3. Significant events
  4. Feedback from patients or those to whom you provide medical services
  5. Feedback from colleagues
  6. Compliments and complaints

The health and probity statements are also required.

What supporting information goes where?

The supporting information is mapped to the GMP domains as follows:

* For the purposes of medical revalidation, QIA, feedback from patients and feedback from colleagues are required once every 5 years.  However, check with your local health boards who may have differing QIA requirements for appraisal.

In the updated GMP 2024 guidance there is more focus on:

  • Compassionate working cultures
  • Effective teamwork and interpersonal relationships
  • Working and training environment free from discrimination, where people are respected and valued as individuals
  • More specific requirements of doctors in formal leadership roles
