Writing the report


You do not need to write a long report, but your report is the only information that your appraiser (or anyone required in the future to inspect your revalidation portfolio) will have, so ensure that it is clear and unambiguous.

  • Choice of topic - provide the reader with the evidence for your choice of topic and explain why you have chosen it.
  • The review - provide the reader with the evidence they need to support your conclusions. Provide references if you have quoted guidance or evidence to support your views.
  • Ensure your plan for change is clear. If you have written a practice protocol, include it.
  • Comment upon how you will know that change has occurred.

A template has been provided below which you may choose to adapt and use for your report.
