Changes to appraisal forms


This section outlines the key changes proposed for the new appraisee version of SOAR.  Download documents below for detail.

  • References to "Form 1", "Form 2", "Form 3" etc will be removed, and replaced with references to supporting information categories instead (to make it more explicit what sections the appraisees are completing)
  • Background information to be simplified
  • Work you do section to be simplified
  • PDP to be simplified
    • "Additional Learning" from PDP Review to be removed (as this can be logged under CPD)
    • "Credits" to be removed as this is also calculated in CPD
    • PDP Year Ahead entries to be simplified also

My current appraisal
Left hand column shows the supporting information needed; middle column highlights personal details, PDP and self-declarations; and right column shows appraisal meeting details.

Log of CPD Activities
Clicking on the CPD green button will take you to this Log of CPD Activities page.  Functionality remains the same as existing CPD Log on SOAR.  Message (in amber) telling users they don't have to use this function if they already use their own log through their college etc.

CPD Supporting Information page
If they click on the "Supporting Documents" tab along the top, it will give them option to upload relevant supporting info (e.g. attendance certs, college logs etc).

CPD Reflections
The "Collective reflection" tab allows appraisees to consider their overall reflections on all their learning.

CPD Guidance page
The "Guidance" tab outlines the revalidation requirements expected for CPD (every appraisal) with links to additional reflective templates and GMC guidance.  Different supporting information will have different requirements, for example:

QIA Guidance page
QIA is only needed once per revalidation cycle, and is advised accordingly on its guidance page.

Significant Events Supporting Documents page
For most of the other supporting information, such as review of Significant Events (image above), Patient Feedback, Compliments etc, they follow a template of uploading files and text box for reflections (similar to current SOAR).
